The hospital survey manuals provide information to assist in the submission of annual surveys.  In each section below you will find instructions, definitions, and/or templates to guide you in submitting the most accurate data.

In addition to the survey manuals below, WHA Information Center has created a Survey Submission and Compliance Manual to assist you in managing the survey roles and registration, understanding the survey calendar and compliance requirements, and submitting an affirmation to attest to the data and complete the process.
